Diabetes is currently an important public health issue worldwide due to its high prevalence, chronic complications and association with obesity. Obesity causes low-grade chronic inflammation characterized by increased secretion of proinflammatory cytokines and activation of innate immune cells. Both innate and adaptive immune responses play an essential role in the progression of type 2 diabetes. Myeloperoxidase (MPO) is one of the most abundant neutrophilic granular protein that plays a key role in modulating the innate immunity and regulation of inflammation. MPO is a marker of neutrophil activation and degranulation and plays a major physiological role in initiating lipid peroxidation by activated neutrophils. It is used as an inflammatory and oxidative stress marker in several metabolic and cardiovascular diseases. Recently, neutrophil to lymphocyte ratio (NLR) has emerged as easy-to-calculate, inexpensive, new marker of inflammation and oxidative stress that reflects the counterbalance of the two complementary components of the immune system. The objective of the current study was to evaluate the activity of MPO levels and NLR in both healthy subjects and in patients with diabetes and to assess their association with glycaemic parameters. The study population consisted of 63 patients with diabetes and 57 age-and-sex-matched healthy controls. MPO activity levels and NLR were significantly higher in patients with type 2 diabetes (MPO- 459.59±20.0; NLR- 2.44 (1.13)) compared to controls (MPO- 252.43±20.7 UI; NLR- 1.79 (0.80)). There was a significant strong positive association between MPO and NLR in all groups (p<0.001; r=0.61). These markers may be used to stratify patients with type 2 diabetes at risk for cardiovascular diseases and diabetes complications.
